Which link should I use, the extranet one?

No. Use the public guest-facing page, which starts with booking.com/hotel/..., not the partner admin page.

Will it include my dates or prices?

Only if the link you paste contains them as parameters. Prices are always shown live on Booking.com, never stored in the code.

Do I need to be a Booking.com partner?

To have a listing, yes. To make the code, no; it simply encodes whatever Booking.com link you give it. This tool is not affiliated with Booking.com.
