MyQRCreate

Google Forms QR code generator

Forms get far more responses when people can reach them from a phone in one scan instead of typing a long address. This tool wraps your Google Form link in a QR code you can drop onto slides, flyers or posters.

  1. Choose a type

  2. Add your content

Open a Google Form instantly from a scan

In Google Forms click Send, choose the link tab, tick Shorten URL, and copy it. docs.google.com/forms links work too. Example: https://forms.gle/AbCdEf123

  3. Customize the design

    Templates

1,413 style presets in 15 categories

    Showing 1-24 of 1,413 templates

Templates are hand-built style presets (colors, gradients, dot and eye shapes), not AI art. They keep your logo, frame, shape and transparency, and every one is dark ink on a light background with at least 4.5:1 contrast. The scan check above still judges your final design.

Who uses a Google Forms QR code

Event check-in and RSVPs

Put the code on the door or invitation so guests register themselves.

Classroom surveys

Show it on a projector slide so students answer on their own devices in seconds.

Customer feedback

Place it on table cards or receipts to collect comments without an app.

Job and volunteer applications

Pin the code to a notice board so applicants can start the form straight away.

How it works

  1. 1

Open your form in Google Forms and click Send.

  2. 2

Switch to the link tab, tick Shorten URL, and copy the forms.gle address.

  3. 3

Paste it into the field here. Links starting docs.google.com/forms are accepted as well.

  4. 4

Download the code and check the form is set to accept responses before printing.

Frequently asked questions

Can people outside my organization fill it in?

Only if the form is not restricted to your organization or to signed-in users. Check the Responses settings in Forms before printing.

What if I stop accepting responses?

The code still opens the form, but it will show a closed message. Because the code is static you cannot redirect it later.

Is the short link or the long link better?

The shorter forms.gle link produces a less dense code that scans more reliably at small print sizes.
